You authorize regularly scheduled charges to your credit card. You will be charged the amount indicated below each billing period. You agree that no prior notification will be provided unless the date or amount changes, in which case you will receive notice from us at least 5 days prior to the payment being collected.
I authorize Alpha Omega Martial Arts or its agents to charge plus a 5% processing fee on the day of each month until I cancel. I understand that this charge will recur every month unless I cancel at least 5 days before the next billing period by written notice. Additionally, I authorize Alpha Omega Martial Arts to charge a registration fee of $20. If I cancel, I authorize Alpha Omega Martial Arts or its agents to charge my credit card a cancelation fee of $25.
