I understand that by submitting this agreement I am authorizing Alpha Omega Martial Arts and its Agents to add a $5 (plus 5% processing fee) to my existing recurring payment agreement each month.

I understand that I am prohibited from storing perishable food items, items which could potentially be deemed unsafe or cause harm to self or others, alcohol, controlled substances or any other item prohibited by law in my locker.

I understand that if payment (for the locker or for the corresponding membership) becomes past due, the locker may be emptied out by Alpha Omega Martial Arts or any person or company acting on its behalf. The items taken out of the locker may be held by Alpha Omega Martial Arts or any person or company acting on its behalf until any payment balance has been satisfied. If payment is not made or if items are not claimed one month after the last due payment, Alpha Omega Martial Arts or any person or company acting on its behalf may sell or donate the items retrieved from the locker.

I understand that the locker may be opened by Alpha Omega Martial Arts or any person or company acting on its behalf for good cause as determined by Alpha Omega Martial Arts or any of its agents.

I understand that I may only use the lock provided by Alpha Omega Martial Arts and that this lock remains the property of Alpha Omega Martial Arts during and after this agreement ends.

I understand that I am not allowed to change the combination of the lock without authorization from Alpha Omega Martial Arts.
